fill out all the appropriate information, and send into the Nurse's office.

Parents/Guardians,
As the weather becomes more conducive for students to have physical education classes outdoors, I would like to remind any of you who have identified your child as having any type of insect allergy/or trees etc. to make sure he/she has an Epipen in the health office. This would provide good assurance that your child would be appropriately treatment in the event of an emergency.
Contact the school nurse if you have any questions.
ATTENTION All Parents: If you have indicated that your child has a severe ALLERGY on the Emergency Awareness Form at the beginning of the school year and he/she uses an Epipen for emergency treatment, PLEASE make sure we have one in the school health office. This is a Department of Public Health and Winthrop Public Schools policy.In the event your child has some allergic reaction to a food group/environmental we need to be able to treat the child while he/she is in school or on a field trip.
The Epipen should be accompanied by a doctors prescription and sent into the health office as soon as possible. Thank You!
ATTENTION:(Grade 6 & 7 Parents/Guardians)
The department of public health requires that all students entering grade 7 in September are required to have documentation that they have completed the following vaccinations. 1) TD Booster or TDAP and 2) 2nd dose of Varivax if the student did not have the chicken pox. Each school district does have an exclusion from school policy when these requirements are not met.
Our goal is to monitor immunizations on the health record to make sure our children are protected. PLEASE review your child's immunization record at your next doctors appointment.
ATTENTION:(Grade 7 Parents/Guardians)
All grade 7 students need to have a current physical on file. Current records allow students who want to participate in any sport acitivity to have the advantage of meeting the requirements.
